IMPORTANT NOTICE

With effect from 1st January 2012, all Holiday or Business travelers to Sri Lanka must have Electronic Travel Authorization (ETA) for entering in to Sri Lanka. Please visit www.eta.gov.lk for more information.

When applying ETA by third parties payments are to be made through the arrangement made in ETA website www.eta.gov.lk  and obtain acknowledgement of ETA application.  Any payments made to other websites or agencies are not valid to process a valid ETA.  Therefore, always ensure that the payments made by accessing to Sri Lanka ETA website and avoid making repayment at the port of entry to Sri Lanka.

General Information

The Sri Lanka Embassy in Warsaw, Poland is accredited to Romania and Bulgaria and able to accept applications/letters in respect of all consular services/matters from the aforesaid countries.

Processing & Issuing time of Visas, Renewal, and issue of Emergency Passport is usually Two working (02) days, provided the application made is in order.

Consular hours : 9.00 a.m to 1.00 p.m. (Monday to Friday)

Visas

Basic: This Embassy is authorized to issue visas for a maximum period of 3 months (90 Days).

  • General Information

Visas are required for persons proceeding to Sri Lanka for purposes such as business, lectures, educational assignments, academic research, and employment, voluntary work in social service organizations, diplomatic missions and other international organizations or to join their spouses in Sri Lanka.
Application forms can be downloaded from the Embassy Web-site or collected at the Consular Division of the Embassy. A request can also be made by post. Necessary arrangement should be made by the applicant himself to receive the Passport after endorsement of Visas.
The status of the visa cannot be changed after arrival in Sri Lanka (i.e Tourist visa will not be converted to Residence/Working visas).

Diplomatic and Official passports must be forwarded by a Note Verbal from the relevant Ministry of Foreign Affairs / Missions
A recommendation from the NGO Secretariat of Sri Lanka and approval from the Controller of Immigration and Emigration are required in case of Foreign nationals who wish to visit in Sri Lanka to work for an NGO.

Journalists/Media Personnel

News Coverage

All journalists/media personnel visiting Sri Lanka should obtain prior visas from the Sri Lankan Embassy in Poland.
Such media teams are advised to submit the visa request well in advance of their date of departure.
All foreign media personnel are required to obtain a Media Accreditation Card issued by the Government Information Department, to engage in media related work on their arrival in Sri Lanka.
Please contact the Public Communications Division of the Ministry of Foreign Affairs, Republic Building, Colombo 01, (Tel: +94 11 53 39 287/+94 11 53 61 952/+94 11 24 37 633) immediately upon arrival in Sri Lanka. Media Accreditation will not be recommended by the Ministry of Foreign Affairs for any media personnel arriving in Sri Lanka without a prior visa issued by the Embassy of Sri Lanka.

Filming of tourism promotional documentaries

Media teams visiting Sri Lanka to film tourism promotional documentaries or on Sri Lanka Tourist Board (SLTB) Familiarization Tours are required to fill the "SLTB selection criteria for journalist/TV crews for Familiarisation Tours' form. (download form)
The Sri Lanka Tourist Board requires at least 30 working days for processing the relevant documents prior to the scheduled commencement of project. Such media teams should also have a ground handling team who will be coordinating the project in Sri Lanka .

Filming of documentaries / feature films

Filming of documentaries and feature films need prior approval from the National Film Corporation and/or Department of Wildlife, Zoological Department where relevant.
Application for a filming project should be submitted to the Embassy with accompanying documents (synopsis of the documentary/feature film, itinerary of the media team in Sri Lanka and filming location/s, duly filled agreement form).

NGO Personnel

Foreign nationals working for an NGO in Sri Lanka should apply for an entry visa.
Request for a visa for the foreign national should be made to the National Secretariat for Non Governmental Organizations, Ministry of Social Services and Social Welfare in Colombo , by the Non Governmental Organizations with which he/she will be working in Sri Lanka . Issuance of visa by the Embassy is subject to approval from the Controller, Immigration & Emigration of Sri Lanka upon the recommendation of National Secretariat for NGO.

Residence Visa

Residence visas are issued by the Controller/Immigration & Emigration in Colombo. Foreign nationals applying for residence visas must obtain the approval of the Controller prior to their departure to Sri Lanka.
To apply for a Residence Visa, the foreign applicant needs to arrive in Sri Lanka on an entry visa issued by the Embassy of Sri Lanka, along with the prior approval of the Controller of Immigration and Emigration.
Visit visas issued to tourists will not be considered for cinvertion into a Residence Visa

Education, Seminars etc :

Application for above visas should be made on Visit Visa Form [Form 'B'] together with the following additional documents:
(a) A letter from the foreign sponsoring organization.
(b) A letter from the sponsoring Sri Lanka institution supporting the study / training programme, workshop etc.

Multiple Entry Visas


Multiple Entry visa is issued to a foreigner who wishes to enter Sri Lanka on several occasions for the purpose of business or investment.

Issuing Authority: The Sri Lanka Mission abroad can issue multiple Entry visa valid for three months - With the prior permission of the Controller of Immigration and Emigration. This can be extended upon arrival when deemed necessary.

Eligibility: Businessman - any foreigner who visits Sri Lanka for the purpose of business promotion, business discussion, consultancy, inspection of quality of goods, installation of foreign purchased equipment etc. on several occasions within a year may be eligible for a Multiple Entry visa.

Investor - any foreigner who visits Sri Lanka for the purpose of seeking investment opportunities, inspecting investment in Sri Lanka etc. on several occasions within a year may be eligible for a Multiple Entry visa.

Period of Validity - up to a one year. Each stay should not be more than three months.

Procedure for Issuance of Multiple Entry Visa

- By the Sri Lankan Mission abroad

Every foreigner who visits Sri Lanka for the purpose of business or investment on several occasions within the overall period of visa can apply for a Multiple Entry visa valid for three months at a Sri Lankan Mission abroad by submitting the required documents personally. Prior Approval should be obtained from the Department of Immigration & Emigration of Sri Lanka.

- By the Prescribed authority of Immigration and Emigration

Any foreigner who visits Sri Lanka for the purpose of business or investment with prior permission of Controller of Immigration and Emigration can apply for an extension of a Multiple Entry visa up to a maximum period of one year by presenting required documents to the prescribed authority of the Visa Division in the Department of Immigration and Emigration personally.

Documents Required
1. Multiple Entry visa application form 'B2' together with Form 'B'.
2. Valid passport of applicant.
3. Three photographs of the applicant. (Bust size approximately 3.5 x 4.5 cm)
4. Documentary proof of purpose of Multiple Entry visit.
5. Evidence of the financial standing of applicant.
6. A written assurance to return to the country of domicile.
7. Finger prints of applicant if necessary.
8. Any other documents the prescribed authority deems necessary.

Ex Sri Lankan nationals

Tourist visa / Visit visa up to a period of three (03) months can be issued for bona-fide tourists including Ex-Sri Lankans, if they wish to obtain visa prior to departure. Extension of the visa beyond three (03) months and up to six (06) months could be granted to Ex-Sri Lankans at the Department of Immigration and Emigration in Colombo .
However, extensions beyond six (06) months for Ex-Sri Lankans will be granted only on valid reasons (medical, family obligations etc.). No visit visa/tourist visa will be extended beyond twelve (12) months. If an Ex-Sri Lankan wishes to stay in Sri Lanka for more than six (06) months they can apply for one (01) year Residence Visa from the Controller, Immigration & Emigration of Sri Lanka.

Persons holding refugee status

A Sri Lankan national holding refugee status and who has a Refugee Travel Document can transit through /visit Colombo .
A foreign national holding refugee status and is in possession of a Refugee Travel Document can be issued a visa to Sri Lanka if their presence in Sri Lanka is considered essential.
Holder of a Refugee Travel Document whose nationality is indicated as "unknown or stateless"; is not eligible to apply for a Sri Lankan visa.
A holder of a Refugee Travel Document whose nationality is not indicated but place of birth is given as being within Sri Lanka is not eligible to apply for a Sri Lankan visa.

Dual Citizenship holders

Holders of dual citizenship do not require a Sri Lankan visa.
The endorsement of a visa exemption will be made by the Controller of Immigration and Emigration of Sri Lanka on production of the travel document of the applicant with the original Dual Citizenship certificate.

Acquisition of Dual Citizenship of Sri Lanka


Eligibility : Ex-Sri Lankans now holding foreign Citizenship or Sri Lankans qualified for grant of Citizenship of a foreign country, who have contributed to the socio - economic development of Sri Lanka.

1. Application form : from the Sri Lanka Mission in the country of residence

2. Documents required : The statements in the application should be supported by an affidavit (Section E) made by the applicant before a Justice of the Peace or a Commissioner of Oaths. No stamp is required.

1. Applicant's Original Birth Certificate and a copy.
2. If the applicant is a citizen of Sri Lanka by registration, the relevant certificate and a copy.
3. If applicant is married, The Marriage Certificate and a copy.
4. Applicant's Foreign Citizenship Certificate/ Foreign Travel Document/ Permanent Residence Visa.
5. Applicant's current and / or previous Sri Lankan Passports and copies or an affidavit.
6. If applying under professional category, applicant's Educational/Professional Certificate and a copy
(Minimum a two year Diploma / Trade Certificate from a recognized institute)
7. If applying under wealth category, original of the documentary proof of applicant's assets such as;
lands or other immovable properties in Sri Lanka worth of Rs. 2.5 Million or above and a copy.
Deed Valuation report extract and Title report for the same should be also submitted.
8. If applying under 2.5 million category original of the documentary proof minimum one year
Fixed Deposit in a bank in Sri Lanka of an amount not less than 2.5 million.
9. If applying under age category, applicants over 55 years of age original birth certificate and a copy should be submitted.
10. If applying under fixed deposit US$ 25,000/- category, original of a fixed deposit certificate and a copy exceeding US$ 25,000 for a period of three years in a bank in Sri Lanka.
11. If applying under fixed deposit US$ 50,000/- category, original of a fixed deposit certificate and a copy exceeding US$ 50,000 for a period of three years in a bank in Sri Lanka.
12. If the documents referred to above are submitted without originals, they should be attested by the Sri Lankan Mission accredited to that county, or the nearest Sri Lankan Mission or Solicitor or a Justice of the Peace of the country in domicile.

Children born outside Sri Lanka to Sri Lankan Parents

Children born outside Sri Lanka to Sri Lankan parents should obtain a visa to Sri Lanka, if their birth has not been registered under Section 5.2 of the Sri Lanka Citizenship Act.
In the event of their birth having been registered under the above Act, they will be considered as Sri Lankan nationals up to the age mentioned in their respective citizenship certificate.
Those children, whose births have been registered under the above Act, are required to submit their original Citizenship Certificate to the Controller of Immigration and Emigration in Sri Lanka in order to obtain an endorsement for visa exemption.

Official / Diplomatic Passport Holders

Diplomatic and Official passport holders travelling to Sri Lanka on official work, will be issued entry visa on gratis basis, based on the Note verbal issued by their respective Ministry of Foreign Affairs.
Diplomatic/Official passport holders travelling to Sri Lanka on official work, must obtain prior visa.

Students

Issuance of visas to students travelling to Sri Lanka for studies, are subject to the approval of the Controller of Immigration & Emigration.
Application for visa must be submitted to the Embassy at least six weeks in advance, accompanied by a letter from the respective Education Institute in Sri Lanka , that the applicant has been admitted for a course of study.

Employment

A foreigner who seeks employment in Sri Lanka should request the prospective employer in Sri Lanka to seek the approval of the Controller of Immigration & Emigration for the visa. Visas for this category will be issued only after obtaining clearance from the Controller of Immigration & Emigration.

Issuance of Sri Lankan Travel Documents

General Instructions

All applicants living in Poland including children who apply for a Sri Lankan Passport must do so in person at the Consular Division of the Embassy with required documents and face an interview. The Consular Division is open between 09.00 am and 1.00 pm.
Applicants who are living in Bulgaria or Romania must personally hand over the application along with the supporting documents to the relevant Sri Lanka Honorary Consulate. In the alternative the applicant should send all the original documents by registered post. The originals will be returned after perusal. Applicants should make arrangements for delivery of the Passport on receipt from the Controller of Immigration & Emigration.
The Embassy forwards the applications to the Department of Immigration and Emigration, Colombo. Passports are issued by the Controller of Immigration & Emigration in Sri Lanka. If the application is correctly filled and all supporting documents are in order, the process will normally take around 8 to 10 weeks to issue a passport.
The Applicant should submit the originals of the required documents together with copies. The originals will be returned after perusal.
In the case of valid passports where all pages are exhausted, a new passport needs to be obtained. The old passport with valid visas if any will be attached to the new passport.
Telephone inquiries relating to the issuance of passports can be made between 14.00 to 15.00 hours to the Consular Division of the Embassy. Telephone No. 0048 22 8538896 /97 from Monday to Friday.

New Passport

Applications for new passports can be made 06 months in advance of the expiry of the current passport.
New passports can also be obtained when there are no more pages for visa endorsements.
Duly completed applications should be forwarded to the Consular Section of the Embassy with the originals of the following documents along with two photocopies.

  • Form K (passport application form)
  • 4 colour photographs [3.5 x 4.5 cm] taken within 3 months (Direct Face)
  • Current Travel Document and 2 sets of photo copies of ID data and alteration pages
  • Original Birth Certificate (Translations are not considered as originals)
  • Sri Lankan National Identity Card
  • Residence Card or documents relating to the present visa status of the applicant, if the visa is not stamped on the passport.
  • For female applicants, in case of the change of the maiden name after marriage, original Marriage Certificate. If the marriage has been registered outside Sri Lanka, such Marriage Certificate should be countersigned by the diplomatic representation of the country where the marriage was registered.
  • If the applicant is a Sri Lankan citizen by registration, original Citizenship Certificate/Dual Citizenship Certificate
  • Academic/Professional Certificate or letter of appointment if the job description is required. If there is a change in the profession or new profession to be included in the passport, original professional certificate or similar proof which has to be certified by the Consular Division of the Ministry of Foreign Affairs, Colombo or the diplomatic representation of the country where such document was issued.

Relevant passport fee

New Passport in lieu of a lost Passport

Application for a new passport in place of a lost passport can only be obtained by calling over personally at the Consular Section of the Embassy.
Required documents

  • Duly filled form 'K'
  • Duly completed form 'D'
  • 4 colour photographs [3.5 x 4.5 cm] taken within 3 months
  • Original Birth Certificate
  • Sri Lankan National Identity Card. If the original of the Sri Lanka National Identity Card is not available, the applicant should submit documentary evidence to satisfy the Embassy of his/her Sri Lankan nationality
  • An affidavit explaining the circumstances as to how the passport was lost. If the affidavit is in Polish or any other languages , it should be accompanied by a translation in English by a sworn translator
  • Original Police report with English translation issued by the Police Station to which complaint was made regarding the loss of the passport.
  • Authorization from the respective government confirming that the applicant has not obtained citizenship/Refugee Status in that country

Relevant passport fee

Travel Document for Minors

Children who are below 16 years of age and whose births have been registered with the Embassy will be included in either the father's or the mother's passport if a request is made to this effect. A separate Travel Document can also be obtained if required.
All original documents should be submitted with two photocopies.
Documents required

  • Form K
  • 4 colour photographs [3.5 x 4.5 cm] taken within 3 months
  • Original Birth Certificate
  • If the child is born outside Sri Lanka, original citizenship certificate issued under the Citizenship Act No. 18 of 1948, or copy of the receipt of payment for Citizenship Certificate, if the Citizenship Certificate has been applied for but has not yet been issued
  • Proof of residence of child/children (Residence Permit/Visa, school attendance card or a letter from the school where the child/children is/are studying in case of school going children.)
  • Travel documents of parents
  • Proof of citizenship of parents
  • Original Marriage Certificate of parents
  • Written consent from parents giving agreement for issuance of a separate passport to the child
  • If parents are divorced or separated an original copy of the court decision / guardianship. Such documents should be countersigned by the relevant authorities.

If a guardian applies for a passport for a child, the guardian must produce the legal documents to prove his/her guardianship.

Travel Documents - Non-Machine Readable Passports (NMRP)

Temporary Travel Documents (Non Machine Readable Passports - NMRP) are issued by this Mission only under special circumstances. NMRP is issued to return to Sri Lanka or to undertake urgent travel pending the receipt of regular travel documents.
NMRP valid for six months may be issued to enable Sri Lankan nationals to return to Sri Lanka only, due to an emergency, in the following circumstances.

  • The original passport is lost / damaged while in Poland, Bulgaria or Romania
  • Emergency situation where travel is essential within a short period, and the Passport has expired
  • Applications for NMRPs should be accompanied by the following documents.
  • Form "IM 36"
  • 4 colour photographs of size 35mm x 45mm taken within 3 months
  • Declaration form (Form D) (if the applicant does not have a passport)
  • Application for addition/deletion of children's names where necessary
  • Original Birth certificate
  • Original Sri Lanka National Identity Card
  • Original Marriage Certificate where applicable
  • Travel itinerary/Return Air ticket
  • Relevant fee and additional charges if the applicant is not in possession of a passport
  • Original Police report with English translation issued by the Police Station to which complaint was made regarding the loss of the passport. This Police certificate should be legalized by the relevant government authority.

Amendments to Sri Lankan Travel Documents

Extension of Validity of Passport

Sri Lankan Passports are issued for a period of 10 years with effect from 13th March 2006. the Passports issued with five (5) years validity can be renewed up to a period of 10 years from the date of the first issue.
Validity of a passport which is over ten years from the date of issue cannot be further extended and the holder of such passport is expected to apply for a new passport.
Extension of validity of passports can be done on the same day, upon submission of the documents to the Embassy by the applicant.
Documents required;

  • Current Passport
  • Duly filled Form 'O'
  • Valid original Resident Permit
  • Sri Lankan National Identity Card

Relevant fee

Inclusion/ deletion of minor/s into/from either parent's travel document

Names and photographs of minors below 16 years of age whose births have been registered with the Embassy and who do not possess a passport can be included in either the father's or the mother's passport, if a request is made to this effect.
The Embassy can include photographs and names of children only in 'L' and 'M' series passports of either parent. The Embassy cannot include children in the new 'N' series passports since their photographs have to be done digitally, and this facility is only available with the Department of Immigration and Emigration in Colombo .
'N' series passports where children's photographs are pasted by the Embassy will be cancelled when they return to Sri Lanka and they will have to obtain new passports by paying the relevant fee. As a result, there could be inconvenience if there are visas stamped in those passports. Therefore, the Embassy will include name/s and photograph/s of children on their parent's 'N' series passport only if the parents are willing to accept the above mentioned conditions.
The names of children that already appear in the passport of one parent may be deleted and included in the passport of the other parent if so required, with the consent of the parents.
No additional fee will be levied, if the inclusion of children is done at the time of applying for a new passport.
Required documents;

  • Duly filled form 'C'
  • 4 colour photographs [size 2.5 x 3.0 cm]
  • Original Birth Certificate/s of the child/children
  • If the child is born outside Sri Lanka, Citizenship Certificate issued under the Citizenship Act ( if the Citizenship Certificate has not yet been issued the original receipt of payment for Citizenship Certificate )
  • Travel Document of parents, if one of them does not possess a Travel Document an Affidavit confirming same
  • A letter signed by the parents giving consent to include/ delete the of name/s and photograph/s of their child/children into/from either parent's travel document
  • Original Marriage Certificate of parents. That certificate should be legalized by the Consular Division of the Ministry of Foreign Affairs of Sri Lanka. If the marriage has been registered outside Sri Lanka it should be countersigned by the relevant diplomatic representative of the country where the marriage was registered.
  • If divorced, the original divorce certificate and documents to prove the legal custody/guardianship of child/children. Those certificates should be legalized by the Consular Division of the Ministry of Foreign Affairs of Sri Lanka. If the divorce has been issued by an authority in a foreign country that certificate should be countersigned by the relevant diplomatic representative of the country where the certificate was issued.
  • In case of widow / widower, the original Death Certificate of the spouse

Alteration of Name

Alterations to name/s may be included in existing passports of Sri Lankan nationals provided that the name/s in the birth certificate has/have been altered in accordance with the birth registration regulations of Sri Lanka . Those birth certificates must be certified by the Consular Division of the Ministry of Foreign Affairs, Colombo .
Female applicants using name of her husband after marriage, who wish to include it in her passport should fulfill the following formalities.

  • Original or notarized copy of marriage certificate issued in Sri Lanka or in a Foreign Country with two photocopies. The marriage certificate should be certified by the Consular Division of the Ministry of Foreign Affairs, Colombo . If the marriage has been registered outside Sri Lanka , the Marriage Certificate should be countersigned by the diplomatic representation of the respective country.
  • Passports of both husband and wife with two photocopies of details pages

Valid original Resident Permit

Change of Employment

A letter from employer or similar proof, which has to be certified by the Consular Division of the Ministry of Foreign Affairs, Colombo or the diplomatic representation of the country where the document was issued.

Include or Change National Identity Card (NIC) number

The original National Identity Card with a certified copy by the Commissioner Department of Registration of Persons of Sri Lanka should be provided. The Certified copy should be authenticated by the Consular Division of the Ministry of Foreign Affairs of Sri Lanka.

Dual Citizenship Endorsement

If the applicant holds Dual Citizenship in Sri Lanka and in Poland , Bulgaria or Romania , the original Dual Citizenship Certificate should be provided.
If a passport has been issued with a restricted endorsement and such endorsement has to be amended or deleted, the passport should be submitted along with relevant supporting documents.

Registration of Births outside Sri Lanka

  • Sri Lankan whose children were born outside Sri Lanka are eligible to register their Births under this section.

Under Section 7 of the Consular Functions Act, the Embassy exercises the power to register a birth of a child born to Sri Lankan parents, in Poland, Bulgaria or Romania.
The application for the registration of birth must be made within three months of the birth.
When a child is born abroad to a citizen of Sri Lanka, there is an additional requirement under the Citizenship Act that such birth should be registered within one year at the office of the diplomatic representation of Sri Lanka in the country of birth or at the Citizenship Division of the Ministry of Deffence in Colombo. Therefore, when an application is made for the registration of a birth under the Births and Deaths Registration Ordinance, an application for the registration of the child under the Citizenship Act should also be made.
A child born to a Sri Lankan parent outside Sri Lanka should obtain the Citizenship Certificate from the Citizenship Division of the Ministry of Internal Administration, Department of Immigration and Emigration at 2nd Floor , 41 Ananda Rajakaruna Mawatha Colombo 10 Sri Lanka.

The applications for the Citizenship Certificates are forwarded to the Citizenship Division in Colombo which will process them and send the Citizenship Certificates to the Embassy.
When an application for registration of a birth is made one year after the birth of the child, such applications are forwarded to the Registrar General's Department at 280, Main Street, Colombo 11 , Sri Lanka.
In such cases the birth certificate is issued only after approval is received from the Registrar General.
The application form for the registration of birth could be obtained over the counter at the Consular Section of the Embassy or by post, sending a self-addressed stamped envelope with a request.
The Birth registration fee of PLN 100.00 (Registration of Birth PLN 25.00 & issuance of Birth certificate PLN 75.00) (additional PLN 15.00 for each year delayed) can be paid at the Consular Division of the Embassy or credited to the Embassy's PLN Account No. 13 1030 1508 0000 0005 0426 5005 at Citi Bank Handlowy, Ul.Senatorska 16, 00-923 Warsaw , Poland.   • Required Documents (the original documents should be submitted with two photocopies)
  • Duly filled Application (Ceylon Citizenship Act , No. 18 of 1948 ( Citizenship 1)
  • Duly filled Declaration of Birth under Section 16 (B4) (if application is submitted within 3 months of the child's birth
  • Duly filled Declaration of Birth after three months Section 24 (B6) (if application is submitted after 3 months of the child's birth)
  • Original Birth Certificate of the child in which the names of both parents appear (Birth Certificate has to be issued by the competent authority in the country where the child was born and should be counter signed by the Consular Division of the Ministry of Foreign Affairs or their diplomatic representative in of the country where the child is born)
  • Original visa card of the child
  • 3 copies of recent photographs of the child (35 x 45 mm)
  • Valid travel documents of the parents with two photocopies of each details pages/photo pages and endorsement pages
  • Resident permit of the Parents
  • Original Marriage certificate of the parents. (If the marriage has been registered outside Sri Lanka it should be countersigned by the relevant diplomatic representative of the country concerned)
  • Citizenship Certificate of parents if they were born outside Sri Lanka
  • If the child's birth was not registered within one year of the birth, a letter signed by both parents, giving reasons as to why it was not registered within the first year of birth
  • If parents are not legally married at time of child's birth, an affidavit relating to paternity and declaration of Citizenship

Upon Registration of the Birth, the first copy of the Birth Certificate would be delivered to the applicant over the counter or by post and the second copy will be sent to the Registrar General's Department, Colombo, for record purposes. The Embassy does not issue additional copies of Birth Certificates. Applicants who wish to apply for additional copies should request them from the Registrar General's Department in Colombo, Sri Lanka.

Registration of Deaths outside Sri Lanka

  • A death will be registered at the Embassy and a death certificate will be issued under the Consular Functions Act, provided the following conditions are satisfied:
  • The deceased is a citizen of Sri Lanka, and
  • The death had occurred in Poland, Bulgaria or Romania.


II. Application for the registration of a death should be made to the Mission within three months of such death on form B11 and late application on form B15, together with the following documents:

(a) The death certificate issued by the Competent Authority of the country in which the death occurred.
(b) Passport of the deceased valid at the time of death. If the deceased is a minor, the passport of the parent in which the endorsement regarding the deceased is made.
(c) If the application for registration of death was made after three months a letter explaining the reasons for such delay must be attached.

III. An application for the registration of death may be made by any one of the following persons:

    • The nearest relative of the deceased who was present at the time of death or in attendance during the last illness of the deceased;
    • A relative of the deceased who resides in the country where the death occurred;
    • If there is no such relative any person who was present at the death;

the main occupant of the house, where the death occurred;

    • The doctor in charge of the hospital or public institution where the death occurred;
    • If there are no such persons, the person who takes action to dispose of the body of the deceased by burial, cremation or by any other means.

Registration of Marriage outside Sri Lanka

  • One of the parties to the marriage should be a Sri Lankan Citizen and should give Notice of Marriage to the Embassy. (Forms are available at the Embassy).
  • The marriage may be registered in the Embassy after a lapse of twelve (12) working days from the date of receipt of the Notice of Marriage and within three (03) months from such date.
  • The Embassy charges a fee of 120.00 for the registration of a marriage and PLN 150.00 for the registration of a marriage under Special License.
  • Valid passports of both parties should be produced.
  • Two (02) witnesses with photo identification documents should also be present.
  • The originals of the following documents are required for the registration of a marriage:

    a. Passport
    b. Unmarried Certificate authenticated by the Consular Division of Ministry of Foreign Affairs of Sri Lanka
    c. Notice of Marriage
    d. Proof of Residence
    e. Fee of PLN 120.00 or PLN 150.00 as applicable can be paid in cash of credited to Embassy's PLN Account No. 13 1030 1508 0000 0005 0426 5005 at Citi Bank Handlowy, Ul. Senatorska 16, 00-923 Warsaw , Poland. Bank Charges should be paid by the applicant. Please make sure to send a copy of bank transfer slip along with relevant documents.
  • Marriage between two Sri Lankan Citizens

Both parties should obtain an unmarried Certificate issued by the Grama Niladari (Village Headman) in the applicant's city/village, which should be countersigned by the Divisional Secretary and authenticated by the Consular Division of the Ministry of Foreign Affairs of Sri Lanka, Colombo.

  • Marriage between a Sri Lankan citizen and non - Sri Lankan Citizen
  • Unmarried Certificates should be obtained by the Sri Lanka Citizen. (Procedure is same as above.)
  • The other party to the marriage is required to obtain a Bachelor Certificate from the Embassy of his/her country.

Other Consular Services

Attestation of Documents

Applications for attestation of documents can be submitted to the Embassy. However if such documents have been issued by any authorities in Sri Lanka they should be certified by the Consular Division of the Ministry of Foreign Affairs in Colombo with respective seals and signature/s .
Document issued by a foreign country should be counter signed by the Ministry of Foreign Affairs of that country or its diplomatic representative in the country in which such document has been issued, before the request is made to the Embassy.
The Embassy only authenticates the seal and signature of the officer who had legalized such document and not the contents of the document.
In the absence of such certification on documents, they would not be accepted for attestation.
Letters/documents regarding obtaining of visas are not attested.
Deeds/titles, bank statements, appointment letters, defamatory documents, etc. are not attested.

Notes

    • This Embassy is not responsible for any document lost in the post.
    • Submission of forged documents and making false declarations relating to citizenship is a punishable offence. Fees will not be refunded in these circumstances.
    • Incomplete applications will be rejected.

Translation of Documents

Only original documents issued by the Government of Sri Lanka and authenticated by the Consular Division of the Ministry of Foreign Affairs in Colombo, Sri Lanka will be translated by this Embassy.
Only the documents that are in English / Polish are translated to Polish/English.

Power of Attorney /Sworn Affidavit

Where an affidavit/ Power of Attorney is required for submission to a Court of Law in Sri Lanka or any other legal institution or matters related to legal implications, the applicant should obtain the affidavit/ Power of Attorney from a lawyer in Sri Lanka and bring such affidavit to the Embassy along with the following documents; and it will be signed in the presence of a diplomatic officer at the Embassy;

  • Affidavit/ Power of Attorney prepared by a lawyer
  • Applicant's valid passport
  • National Identity Card
  • Resident permit
  • Signatures of two witnesses who hold valid passports

Relevant Fee

Police Clearance Certificate

Applications for police clearance can be forwarded to the Police headquarters in Colombo through the Embassy.
Sri Lankan nationals living abroad may submit their request for a police clearance certificate directly to the Inspector-General of Police, Police Headquarters, Colombo 01, Sri Lanka if they wish to do so. In such cases the Police Clearance Certificate would be sent directly to the applicant.
